By checking this box, you agree to our Release of Liability & Indemnification (required): I have voluntarily chosen to participate in the dance training offered by DANCE EXPRESS. I am aware that dance training and related activities carry certain risks that can result in injury, both minor and major and I am fully aware of the risks and hazards involved. I have received advice from a healthcare provider regarding my ability to participate in dance training and related activities. If my children are participating, I am aware of their physical limitations and agree not to exceed them.
I, on behalf of myself and my heirs, guardians, personal representatives, and assigns, do hereby release, indemnify and hold harmless DANCE EXPRESS, its employees, teachers, dancers, staff, and Board of Directors from any and all liabilities, claims, demands, and causes of action, arising out of or relating to any personal injuries (including death) and property losses or damages that may be sustained by me or my children in connection with any activities occurring in my home, premises or any other location I choose when engaging in learning opportunities with DANCE EXPRESS (the “Release”).
I expressly agree that this Release is intended to be as broad and inclusive as permitted by the law. I agree that if any portion of this Release is held invalid, the remainder shall, notwithstanding, continue in full legal force and effect.
I have read the above, fully understand its contents, and voluntarily agree to its terms and conditions. By checking "I agree" below or in any way utilizing the online content provided by DANCE EXPRESS, I am legally accepting the DANCE EXPRESS remote learning policies and participation waiver and this Release. If I am signing this Release on behalf of my child, I certify that I am their parent or legal guardian and have the right to sign on their behalf.
